The GAIKOKUJIN-TOROKU (alien registration) clarifies a place of residence and other relations for foreigners.
There are the following kinds of alien registration applications. An application period and required things are dependent on the kind of application.

National Health Insurance (Kokumin Kenko Hoken)
All residents of Japan are required to join a public health insurance scheme. There are two types: National Health Insurance (Kokumin Kenko Hoken) and Employee Health Insurance (Kenko Hoken).
Foreign nationals who have completed Alien Registration and have a status of residence of at least one year must enroll in one of these schemes.
Unless you and your family are already covered by Employee Health Insurance, you must enroll in the National Health Insurance scheme, which is administered by the local authority.
If you have public health insurance, you will only need to pay 10% - 30% of the total medical cost for treatment of illness or injury. (The cost varies according to age and income.)
To enroll in the National Health Insurance scheme, you must go to the National Health Insurance Section at Kamakura City Hall.
Please note: Branch offices do not carry out health insurance enrollment procedures for foreign residents.

When leaving Japan, please return the certificate of alien registration at an airport or a harbor. (However, it is not necessary if you have the reentry permit certificate)
If Japanese nationality is obtained, the principal has to return the card within 14 days of that day.
If someone in your family dies, relative who lives with principal has to return within 14 days of that day.

The JYUMIN-HYO (resident record) is the certification of citizen's personal data such as name, address, birth date, and permanent domicile. It is also filled in some closely related information for daily life such as polling to a voter registration, affiliation status of national health insurance and/or national pension.
When you move, the JYUMIN-IDO-TODOKE (change report of residents) should be submitted, the notice also includes the information such as suffrage, national health insurance, and school attendance of elementary school and junior high school.
If the notice is not submitted, it means impedimenta to things such as use of the suffrage, school attendance, and grant of national health insurance. So, please submit as soon as possible, referring to the following tables.

The KOSEKI (family register) is an official identification system through which a married couple's births, parents, marriage, marital status, and deaths are registered.
The record of one's address at registration is called the HONSEKI (permanent domicile).
The KOSEKI is maintained at the office where you place HONSEKI.
Although the copy/abstract of KOSEKI can be taken only in the office you place HONSEKI, the notification about KOSEKI can be performed at one's residence address.
Please refer to the following table. However, please take notice that it is necessary to submit a copy/abstract of KOSEKI when submitting notice except at the office you place HONSEKI.
If you submit the notification documents to Kamakura city office, your trouble is quit for one copy even the documents related to some municipalities.
The notification form is common to all municipalities. In most of the case, the person who actually submits the notification documents may be substitute.

The notification documents are also in a hospital. Since the right half is a birth certificate, please obtain the proof from a doctor or midwife. Child's name should be used commonly used Chinese character, a name-of-a-person Chinese character, a hiragana, and katakana (If a child has a foreign family register such as South Korea, Korea, and the China, then it should be used a Chinese character. In other case, it should be used katakana). Also, the procedure in other sections in its duty may be required, such as a child allowance and aid of medical expenses. |

You have to get signatures and seals of two witnesses (those who have reached adult age) on the right half of marriage certificate. If a marriage registration is not taken out, even if it goes into married life, you are not accepted as formal husband and wife legally. Since a marriage registration is the notification of a family register, if you also move the address with marriage, please submit a residents change report. If either has a foreign family register, other documents are required. Please call the office. |
